Paso Del Norte Health Foundation Awards $422,692 Grant

The University Medical Center Foundation of El Paso was awarded a $422,692 grant from the Paso del Norte Health Foundation to support the launch of Diabetes Care Plus, a three-year care management program aimed at improving diabetes treatment adherence among patients.

The initiative, which will be piloted at the UMC of El Paso Northeast Outpatient Clinic, is designed to assist patients struggling to manage their type II diabetes effectively. With the funding, a clinical pharmacist will work in close collaboration with physicians to coordinate patient care, provide individualized education, and ensure ongoing follow-up.

"This initiative is designed to assist diabetes patients who are struggling to manage their condition effectively," a UMCF spokesperson stated. "With this grant, a clinical pharmacist will work closely with physicians to coordinate patient care, provide personalized education, and ensure thorough follow-up, all aimed at improving patients, adherence to their treatment plans."

To enhance patient outcomes, officials are establishing a Collaborative Practice Agreement to formally define the roles and responsibilities between physicians and the clinic’s pharmacist. This framework will facilitate more effective drug therapy management, patient education, and healthcare provider coordination.

UMC of El Paso currently provides care for over 11,000 type II diabetes patients, many of whom face challenges in maintaining their treatment regimens.

"Diabetes Care Plus will enable UMC of El Paso to implement evidence-based care management strategies for individuals facing challenges with their diabetes treatment plan," said Dr. Juan Castillo, the attending endocrinologist for the grant program.

With this grant, UMCF continues its commitment to enhancing healthcare services in the region and ensuring that patients receive the support they need to manage chronic conditions effectively.
